Immunizations and physical forms on the portal

With summer approaching and summer camps gearing up, and summer jobs and internships starting, one common request we get is for copies of immunization records and school forms. These can be obtained right from the patient portal at any time! However, it’s not obvious at first glance how to get them, so we thought a primer would help!

For immunizations:

• log into the patient portal

• in the left side navigation, pick “Chart”

• for the “include information from” drop down, pick “entire history”

• scroll down and check the box for “immunizations”

• scroll down further and you can choose to view, download the document, or email it

School and daycare forms are generated at the time of a well child exam and sent directly to the portal, and are valid for 12 months, so you should get a message about it, which you can follow to retrieve it. However, you can also find them under the “Documents” tab in the left hand navigation (titled “NYS School Health Exam Form” or titled… wait for it… “Day Care Form”)
